CHA is recruiting a Senior Medical Director of Population Health Management Outcomes. As an integral part of the Population Health Management leadership team the incoming Senior Medical Director will provide oversight and guidance for population health performance goals and objectives.
- Work closely with the CMO and population health team to set priorities
- Collaborate with clinical providers, staff, and others to improve utilization, quality of care, patient experience, medical management, and medical documentation
- Design and implement population health initiatives to achieve improved health outcomes and lower total cost of care
- Identify and drive organization and cultural change needed to adapt strategically to changing market demands, technology, and internal initiatives
- Senior Medical Director will provide part time direct clinical care
Ideal candidates will be Board Certified in Family Medicine, Internal Medicine, or Internal medicine/Pediatrics and have 3+ years of experience in a clinical leadership role. Candidates should possess a strong desire to provide the highest quality care to our diverse patient population.
